Appointment Requests

Submitting an appointment request through the website does not guarantee an appointment time. A Healthwise Diagnostics team member may contact you to confirm details, request missing information, provide preparation instructions, or offer available appointment options.

You are responsible for providing accurate contact information, selecting the appropriate service where possible, and ensuring your requisition form is complete and valid.

Requisition Forms and Uploaded Files

If you upload a requisition form or other document, you confirm that the information is accurate and that you are authorized to submit it. Uploaded files must relate to your appointment request and must not contain unlawful, harmful, or unrelated content.

Patient Responsibilities

• Bring your requisition form and valid Ontario Health Card to your appointment.
• Follow all preparation instructions provided for your exam.
• Arrive on time and allow enough time for registration.
• Provide at least 24 hours' notice when cancelling or changing an appointment whenever possible.
• Tell the technologist before your exam if you are pregnant, may be pregnant, or have a relevant medical concern.

Payment and Coverage

Many diagnostic imaging services may be covered by OHIP with a valid Ontario Health Card and medical requisition. Some exams or services may not be covered. If you do not have valid coverage, payment may be required by cash or cheque at the time of your appointment.

Communications Consent

When you submit an appointment request, you consent to receive transactional communications from Healthwise Diagnostics by phone, SMS, or email related to your appointment request, including scheduling, preparation instructions, missing information, and administrative follow-up.
